SIOUX FALLS, S.D., Nov. 10, 2022 — John Snyder, president of Sanford Health Plan and Emily Griese, PhD, chief operating and population health officer at Sanford Health Plan, will present at the Becker’s CEO + CFO Roundtable Nov. 9 and 10 in Chicago. The roundtable brings health care leaders from across the country together to share ideas and exchange best practices on improving patient and member care as well as enhancing operational efficiencies in health care.

Both leaders will give a presentation together called “Creating Virtual Preventive Care Programs for Risk Adjustment,” where they’ll share details behind Sanford Health Plan’s risk adjustment strategy that leverages virtual care.

“Our goal is to facilitate access to the right care, at the right time in the right place,” said Snyder. “We want to improve care outcomes for our members, while reducing emergency room and hospital visits. By taking a creative approach to identifying and engaging our patients and health plan members, we have been able to break down barriers to care, especially those barriers in rural parts of the country.”

In addition, Griese will join a panel focused on population health, where she will share how Sanford Health Plan is uniquely positioned to meet the broad needs of the communities it serves across the upper Midwest.

“The new wave of population health is empowering our patients and members to take control of their health,” said Griese. “We are adopting a proactive data-driven approach to keeping people healthy. By combining demographic, claims and clinical data in a much more predictive way across an entire population, we can better manage chronic conditions and improve the overall health of the members we serve.”

Snyder will also sit on a panel centered around identifying and tackling roadblocks in risk-based contracting. He will join health care leaders from Oregon Health & Science University, Intermountain Healthcare and American Medical Group Association Consulting to discuss the economic challenges health care organizations have faced the past few years.
