Sports physical events in Williston for student-athletes

WILLISTON, N.D. – Sanford Health is offering special walk-in events for student-athletes to complete their sports physicals for the 2025-26 school season. Student-athletes are required to get an annual sports physical when participating in school-sanctioned sports and must have their physicals completed before the first day of practice for their sport.

Student-athletes can receive their sports physical this summer at Williston High School, located at 4200 32nd St. W., without an appointment on the following days:

The cost for sports physicals is $35. A bill for the service will be sent post-event. Parental permission is required to receive a sports physical. Please bring your signed state sports physical form.

If unable to attend a sports physical event at Williston High School, families may  request to schedule a well-child or sports physical appointment with Jolynn Azure, MD, family and sports medicine, or Ndu David Ugobi, pediatrics at Sanford 11th & Gate Clinic, 1700 11th St. W., Williston. These physicians travel to Williston from Minot the third Tuesday of each month.

A well child visit includes the sports physical exam plus growth and development checks, screening for mental health concerns and immunizations as needed. This comprehensive visit is typically covered by insurance.

Student-athletes are asked to bring their school physical form to their visit with parent portion completed and signed. Both traditional sports physicals and well-child visits are by appointment only.

As an extension of sports medicine care in Williston, Sanford Health also provides athletic training services for athletes in the community via Matt Monroe, a full-time athletic trainer.
