Sports physical events in Minot for student-athletes

MINOT, N.D. – Sanford Health is offering walk-in sports physical events for student-athletes to complete their sports physicals for the 2025-26 school season. Student-athletes are required to get an annual sports physical when participating in school-sanctioned sports and must have their physicals completed before the first day of practice for their sport.

Student-athletes can receive their sports physical this summer at Sanford Northwest Clinic in Minot, located at 1500 21st Ave. NW, without an appointment on the following days:

The cost for sports physicals is $35. Parental permission is required to receive a sports physical. Please bring your $35 payment and signed state sports physical form.

If unable to attend a sports physical event at Sanford Health, families may also request to schedule a well-child or sports physical appointment for their young athlete’s annual sports physical.

A well-child exam also offers an opportunity to talk about nutrition, safety, and other age-appropriate topics. Parents are encouraged to check with their insurance provider, as many pay 100% for a wellness exam.

Student-athletes are asked to bring their school physical form to their visit with parent portion completed and signed. Both traditional sports physicals and well-child checks are by appointment only.
