Sanford Health app feature connects patients, providers

SIOUX FALLS, S.D., May 22, 2020 — My Sanford Chart, an online and app-based system that gives patients personalized and secure access to their medical records and Sanford Health experts, is adding an enhancement to better connect patients and providers. The new Our Notes feature is a simple way for patients to ask questions or update their provider.

With a few simple questions, patients can enter details about what has changed about their health history since their last appointment. Our Notes also allows a patient to raise health questions they want addressed in their upcoming appointments, and gives patients an opportunity to start a conversation with their provider before they walk in the door.

“I often have moms come to appointments with a notebook and list of questions,” said Tokunbo Akandé, M.D., a Sanford Health pediatrician. “Now, those questions can easily be entered into Our Notes, and the notebook can stay at home. It really is a simple way for patients to get the most out of every appointment.”

Once a patient enters questions, concerns or goals for the appointment in Our Notes, those details are available to the provider. Not only does this capture each patient’s unique perspective, but also organizes each appointment to meet the expectations of the patient.

“Sometimes, raising a question in person can be difficult for a patient,” said Akandé. “Some questions can be embarrassing to ask in person, or an important question can slip from the mind. Bringing that question into Our Notes ensures it is addressed in the appointment. I like that I can truly capture the patient’s own words.”

My Sanford Chart can also be used to schedule video visits and e-visits for care. For e-visits, patients complete an online questionnaire about symptoms and a Sanford provider will respond within two hours with a treatment plan or prescription, or instruction to be seen in-person.
