GSS nursing leader to share insights at national conference

SIOUX FALLS, SD
– DeeAndra Sandgren, chief nursing officer at the Good Samaritan Society, will advance the national dialogue around resident safety and care transitions this week during the 2024 Skilled Nursing News RETHINK conference.

During the two-day conference, held Sept. 11 – 12 in Chicago, Sandgren will be a panelist for the following discussions with other long-term care leaders across the country:

  • Breaking the Bottleneck: Referrals, rehospitalizations and rethinking how acute care and Skilled Nursing Facilities relate
  • Acing the Survey: Achieving success in a punitive environment for nursing homes

“The safety and care of our residents is our top priority.” said DeeAndra Sandgren. “As part of an integrated health system with Sanford Health, we are better positioned to identify and meet residents’ unique clinical needs, improve in-place access to care and facilitate seamless transitions across care settings. The collaboration and innovation driving these outcomes is a game-changer for our seniors because it reduces disruptions through their health care experience.”

Sandgren will share insights into how the Good Samaritan Society is equipping leaders and caregivers with tools and training to continue advancing safety and quality care for its residents. Over the last two years, the organization has been enhancing its culture of safety through Sanford Accountability For Excellence (SAFE), a high reliability program focused on empowering caregivers to speak up, raise concerns and understand the positive impact of their actions.

She’ll also share how the Good Samaritan Society and Sanford Health collaborate to ensure patients and residents receive the right care at the right place and time.

In 2021, the Good Samaritan Society opened a home hemodialysis den in a Sioux Falls, South Dakota skilled nursing facility – the first of its kind in the state – which allows residents to access Sanford Health dialysis services without leaving home. The model is so impactful for residents that it’s expanding to Bismarck, North Dakota in early 2025.

Also underway is Good Samaritan Society – Founder’s Crossing, a full continuum of care senior living community in Sioux Falls, South Dakota that will include a direct connection to a Sanford Health clinic and Lewis Drug. Through Sanford Health, residents of the community will have convenient access to health care services like lab, primary care, radiology and pharmacy.

Sanford to end contract with Humana Medicare Advantage

SIOUX FALLS, S.D. —Sanford Health will end participation with Humana Medicare Advantage on Dec. 31 due to ongoing challenges and concerns that negatively affect patients including ongoing denials of coverage and delays accessing care.

Sanford Health has notified patients with Humana Medicare Advantage coverage about this change and encouraged them to consider other plan options. Patients can select a new plan during the Medicare Open Enrollment Period, which runs from Oct. 15 through Dec. 7. New plan information for 2025 will be available Oct. 1.

“This is a difficult decision, but ending our partnership with Humana Medicare Advantage is the right thing to do for our patients,” said Martha Leclerc, vice president of corporate contracting for Sanford Health. “We have attempted to work with Humana for several years, but unfortunately, we have continued to experience delays in patient care, barriers to scheduling and denials of coverage causing financial burden and undue stress to our patients.”

This termination does not include any Good Samaritan Society locations at this time. They will continue to be in-network for patients with Humana coverage.

Sanford Health will provide care to all patients, regardless of insurance. Humana-insured patients who choose to stay with their current plan after Jan. 1, 2025, can still receive care at Sanford. However, Sanford Health will be considered out-of-network which will increase out-of-pocket costs for these patients in 2025.

Sanford Health will be in Humana Medicare Advantage’s network through Dec. 31. If a patient has a procedure or appointment in 2025, it will be important for the patient to check their 2025 Medicare plan coverage network to make sure their Sanford provider, surgery center or hospital is included. If patients aren’t sure, they should ask the insurance company before enrolling.

FARGO, N.D.Sanford Medical Center Fargo received a 5-star rating by the Centers for Medicare & Medicaid Services (CMS). Sanford Fargo is the only health care organization in North Dakota to receive a 5-star rating.

This national distinction recognizes Sanford Health’s quality measures and its continued effort in providing safe, quality health care to its patients. This designation was earned by only 8.2% of the more than 4,600 hospitals ranked for quality measures and evidenced-based safety practices.

“We are proud to be amongst the top performing hospitals in the nation for quality,” said Tiffany Lawrence, Sanford Fargo president. “This rating reflects the hard work and dedication to quality and safety by all of our employees providing world-class health care close to home. The safety of our patients and the high-quality health care they receive is our top priority.”

The overall star rating for hospitals summarizes quality information on important topics, like readmissions and deaths after heart attacks or pneumonia. The overall rating shows how well each hospital performed on an identified set of quality measures compared to other hospitals in the United States.

The CMS quality ratings condense a variety of measurables in key areas. The five measure groups that are scored for each participating hospital include:

• Mortality
• Patient experience
• Readmission
• Safety of care
• Timely and effective care

Last month, Sanford Medical Center Fargo was named the U.S. News & World Report’s Best Hospital in North Dakota for the fourth consecutive year.

SIOUX FALLS, SD – Sanford USD Medical Center has received the American College of Cardiology’s NCDR Chest Pain  ̶  MI Registry Platinum Performance Achievement Award for 2024. Sanford USD Medical Center is one of only 259 hospitals nationwide to receive the honor.

The award recognizes Sanford Health’s commitment and success in implementing a higher standard of care for heart attack patients and signifies that Sanford USD Medical Center has reached an aggressive goal of treating these patients to standard levels of care as outlined by the American College of Cardiology/American Heart Association clinical guidelines and recommendations.

To receive the Chest Pain  ̶  MI Registry Platinum Performance Achievement Award, Sanford USD Medical Center has demonstrated sustained achievement in the Chest Pain  ̶   MI Registry for two consecutive years (2022 and 2023) and performed at the highest level for specific performance measures. Full participation in the registry engages hospitals in a robust quality improvement process using data to drive improvements in adherence to guideline recommendations and overall quality of care provided to heart attack patients.

“It is an honor to award Sanford USD Medical Center with the Platinum Performance Award for their valuable national leadership and dedication to meeting comprehensive performance measures in patient care,” said Michael C. Kontos, MD, FACC, chair of the NCDR Chest Pain – MI Registry Steering Subcommittee, and cardiologist at Virginia Commonwealth University Medical Center. “The receipt of this award indicates that Sanford Health remains committed to providing top quality, guideline-driven care for heart attack patients. Their success ensures patients are receiving the highest quality cardiovascular care.”

The Centers for Disease Control estimates that over 800,000 Americans suffer a heart attack each year. A heart attack occurs when a blood clot in a coronary artery partially or completely blocks blood flow to the heart muscle. Treatment guidelines include administering aspirin upon arrival and discharge, timely restoration of blood flow to the blocked artery, smoking cessation counseling and cardiac rehabilitation, among others.

“Sanford Health is committed to ensuring heart attack patients receive the best possible care,” said Bridget O’Brien-Johnson, vice president, operations, heart & vascular. “This honor from the American College of Cardiology is a testament to our providers and our leadership. Standards and guidelines are only as good as the people who put them into practice and the team at Sanford USD Medical Center has consistently proven they are committed to the highest quality of care.”

Chest Pain  ̶  MI Registry empowers health care provider teams to consistently treat heart attack patients according to the most current, science-based guidelines and establishes a national standard for understanding and improving the quality, safety and outcomes of care provided for patients with coronary artery disease, specifically high-risk heart attack patients.

SIOUX FALLS, S.D.
Sanford USD Medical Center has received several recognitions from Newsweek as part of their Best Hospitals 2024 rankings. The annual list recognizes the institutions that have achieved the highest reputations by delivering quality care with outstanding results.

Sanford USD Medical Center is the only hospital in South Dakota to be named to the following lists:

The medical center was also recognized in two other categories, earning a spot on the list for:

“Being recognized as one of the best in the country in these services is a true testament to the talented and dedicated team of caregivers we have at Sanford Health,” said Paul Hanson, president and CEO of Sanford Health in Sioux Falls. “As our region continues to grow, we are recruiting additional providers, nurses and caretakers in several areas throughout our medical center to ensure we continue meeting the community’s health care needs. Patient care is at the heart of what we do every day. Our commitment to the communities we serve is unmatched, and I’m proud to walk alongside the great family of caregivers we have at Sanford Health.”

Rankings are based on three pillars:

  1. Nationwide Online Survey: Health care professionals and hospital managers with knowledge about maternity processes (e.g., neonatal and perinatal doctors, nurses and midwives were asked to recommend leading maternity hospitals in the U.S.).
  2. Patient Experience Survey: Survey of the overall patient experience in the hospital.
  3. Hospital Quality Metrics: Indicators relevant to care provided for specific services were considered.

Earlier this year, Sanford USD Medical Center was recognized as the No. 1 hospital in South Dakota by U.S. News & World Report and was named to Newsweek’s America’s Best Maternity Hospitals 2024 list.
